Make a Referral

We are currently being funded by the South West Regional Improvement and Innovation Alliance (SWRIIA) which means we can only support those living in the South West of England in: Cornwall, Devon, Dorset, Gloucestershire, Somerset and Wiltshire. Please see below for a full list of the specific Local Authorities within this area supported by us. We are working hard to expand our funding so that we can extend our work across the UK. Unfortunately, for now, we can only support those living in this area. We are sorry if that does not apply to you.

 

Our support is offered to parents and carers who are supporting a child / children who are under 18 years old. We are not currently being funded to support foster carers. If you do not qualify for our support due to not supporting a child or living outside the area please head to our Resources page where there is a list of other national organisations that we hope will be able to help you.

We have two referral forms one for professionals working with a client for example those in the police, GPs, social workers and Independent Sexual Violence Advisors and also one for self-referrals if you  want to refer yourself in and receive our support.

These referral forms are for our Trauma Support Programme which is a one to one, trauma informed support programme offered either face to face, online or via the phone. It is offered for an hour each week for 10 consecutive weeks.

Referral Forms

Click on one of the forms below, fill in and email to support@actsfast.org.uk. After we receive your referral form we will send a confirmation email saying that we have received it, this is usually within 72 hours, and if you are eligible for our support we will then offer you an assessment where we can find out more about you and make sure our Trauma Support Programme is the right service for you.

Once this form is filled in and submitted it will be sent to our support@actsfast.org.uk email address and we will let you know within 72 hours that it has been received. If you have not heard from us in this time please feel free to email us and check if it's been received.
